Residents alerted police to the fact a man was swimming in the Wollondilly River near the McDermott Drive bridge just before 6pm Thursday.
Officers called in a Goulburn SES unit with their flood boat for assistance as he refused to come out, police said.
The man, Daniel Todd, was uninjured when they retrieved him from the water and was safely returned to the Chisholm Ross Centre.
He had gone missing after being allowed an unsupervised walk at 10.30am last week.
A facebook update on his health posted late on Thursday night said:
"The first information that we received was that Daniel was been treated for some sunburn and would be moved back to Chisholm Ross Hospital. We have just finally spoken with the treating Doctor at Emergency Department in Goulburn Base Hospital. Daniel is very unwell physically, He has a chest and skin infection, he is also quite dehydrated. He is currently in intensive care and will be moved to a general ward once his physical condition improves."
