Police said they were called to the casino at 1.30am on Sunday to reports of an alleged assault on a patron.
The patron, a 32-year-old French national, told pop culture website pedestrian.tv he had his right leg broken in two places when up to five security guards wrestled him to the ground while on a night out with friends.
He said the incident was sparked after he started filming on his phone the removal of one of his mates from the casino who had been caught drinking out of a champagne bottle.
The man said security slapped his phone out of his hand and when he went to pick it up he was knocked to the ground.
The man said he was then escorted through the casino by security and was tackled to the floor near the entrance, causing a double fracture in his leg.
"I felt it break," he told pedestrian.tv
Another friend filmed this incident which was then uploaded by pedestrian.tv.
The Star told Fairfax Media it was conducting an internal review into the incident and said the safety of its guests was of paramount importance to the venue.
A spokeswoman said the incident concerned a man who had become involved when security approached another member of his group who had "reportedly directed abuse towards a food and beverage attendant".
"The abuse towards staff followed repeated requests to pour the contents of a glass bottle into plastic cups, in accordance with our RSA [responsible service of alcohol] procedures," a statement said.
"These requests were ignored.
"The behaviour of concern included foul language and a security supervisor stated he was assaulted. Requests to leave the venue were also ignored."
A NSW police spokeswoman said the 32-year-old man attended Sydney's Day Street Station at 10pm on Sunday and formally reported the incident.
Police were now investigating, she said.
